Shows an employee’s time-off request form with leave type chips, from/to dates, half-day toggles, a note for the manager, the shifts that would be released (one needs cover) and a summary with the balance before and after. Corporate style: IBM Plex Sans, a dark brand-coloured sidebar, 4px corners and subtle shadows. Comes with the full HTML and a DESIGN.md with light and dark colour tokens.
