Time-off requests go both ways: an employee asks, a manager decides. These examples show a request form with leave type, dates, half-day options, a note and the shifts it releases; the same form when a day would drop below minimum staffing; and a manager’s review with a coverage check and approve or decline buttons. Every example is an original design that comes with its HTML code and a DESIGN.md of colour, type and spacing tokens, in light and dark. Free to use and adapt.

Explain which day and shift would be short, mark the date fields, and offer a way forward, such as asking a named colleague to cover or moving the dates, rather than just blocking the request.
Who is asking, the type and dates, the balance afterwards, the shifts affected, whether each one is covered, and any overlapping requests.
