Make end call large, distinct and always in the same place.
Show the call timer and connection quality so people know the call is live.
Keep mute and speaker one tap away, with clear on and off states.
On desktop, use the extra space for context about the person, such as recent messages or shared files.

Questions

What controls should a call screen have?

Mute, speaker or audio output, keypad, add a person, hold and end call. Video calls add camera on or off and screen sharing.

How should an active call appear when the user leaves the call screen?

Keep a small persistent indicator, such as a bar or floating pill with the timer and end and mute buttons, so people can return to the call or hang up from anywhere.
