A contact sales form turns interest into a conversation, so it should be quick to fill in and clear about what happens next. Our examples include name, work email, company, a team-size select and a message field, alongside separate routes to sales, support and a status page, designed in eight visual styles for web and mobile. Every example is an original design that comes with its HTML code and a DESIGN.md of colour, type and spacing tokens, in light and dark. Free to use and adapt.

Keep it to what you need to respond well, typically name, email, company and a message, plus one qualifying question such as team size. Each additional field adds effort. Mark optional fields clearly, or better, remove them and gather that information during the follow-up conversation.
Only if you will genuinely call, and preferably make it optional. Many people are reluctant to share a phone number with a company they have not spoken to yet, and a required phone field can deter enquiries. Email is enough to start the conversation.
Show a clear confirmation on the page that repeats the email address used, says who will reply and gives an expected response time. Send a confirmation email too. Offer useful next steps, such as documentation or booking a call, while they wait.
