How to make a Secret Santa questionnaire
- Choose a question set: work exchange, family, friends or kids. Tick “Add the fun extras” for a few lighter ones.
- Untick questions you don’t want, edit the wording, move them up or down, or add your own.
- Add the event, budget and swap date. They print at the top of the form.
- Pick a design, the paper, one or two columns and the heading font.
- Download the fillable PDF and share it, or print it at 100%.
- To answer it yourself, open “Fill it in here”, then save the answer card or a PDF with your answers.
What you can use it for
- An office Secret Santa where people barely know each other
- A family exchange with sizes and wish lists
- A friends’ swap with a few fun questions
- A class or club exchange with a short kids’ form
- A remote team that fills in forms on their phones
- Sending your own answers to the organiser as a picture
The question sets
Each set is a starting point. Switch questions off, edit them or add your own. Tick boxes are the this-or-that questions, and some questions have extra lines or size boxes.
| Set | Questions | Tick-box questions | Extra room | First three questions |
|---|---|---|---|---|
| For a work exchange | 10 | 2 | 1 | Coffee, tea or something else? How do you take it?; Favourite snack to keep at your desk; Sweet or salty? |
| For family | 10 | 1 | 2 | Three things you’d love this year; Sizes, only if you’re happy to share; Where to find your wish list |
| For friends | 10 | 1 | 1 | Something you’d love but wouldn’t buy for yourself; Scents you like, or can’t stand; Your go-to drink |
| For kids | 8 | 1 | 1 | Favourite colour; Favourite animal; Favourite game or toy |
| Fun extras | 6 | 3 | 0 | Cosy night in or big night out?; Cats or dogs?; Mountains or the beach? |
What makes a good Secret Santa question
- Ask about tastes, not prices: favourite snack, colour, scent or shop.
- Give a few quick this-or-that choices; they’re easy to answer and useful.
- Always ask about allergies and things to avoid.
- Keep sizes for family and close friends, and make them optional.
- End with one wish, so the giver has a sure thing to fall back on.
For a work exchange, keep it light and about the desk, the coffee order and hobbies. For family, a wish list link and sizes save guesswork. For kids, short questions with room to write or draw work best.
Filling in and sending answers
The PDF works three ways. Fill it in on a phone or computer and send the saved file back. Print it and write by hand. Or open this page, fill it in under “Fill it in here”, and save a 1080 × 1350 px answer card, a size that suits a chat message, or a PDF with your answers already in the fields.
| Download | What it is | Size |
|---|---|---|
| Fillable PDF | The blank form with a field for every answer and tick boxes | US Letter 215.9 × 279.4 mm or A4 210 × 297 mm |
| PNG | Each page as a picture, for printing or sharing | 300 DPI: 2550 × 3300 px on Letter, 2480 × 3508 px on A4 |
| Answer card | Your filled-in answers on a design, as a picture | 1080 × 1350 px (4 × 5 in at 270 DPI) |
| PDF with my answers | The form with your answers already in the fields, still editable | Same paper as the form |
Tell people who gets the answers: usually the organiser, who passes each one to the right Secret Santa. Share answers only with the giver and the organiser, not the whole group. The designs are our own drawings: you may use them and anything you make with them, including things you sell, with no credit needed.

Made Good Designs isn’t connected with any form or Secret Santa service. Every picture is our own drawing.

Frequently asked questions
What should a Secret Santa questionnaire ask?
Favourites, a few this-or-that choices, things to avoid, allergies and one wish cover most groups. Family forms often add sizes and a wish list link, and work forms stick to desk, snack and coffee questions.
How many questions should it have?
Short forms get finished. Eight to ten questions suits most adults, and the kids’ set has eight. You can switch any question off.
Is the PDF really fillable?
Yes. Every answer is a form field, and the this-or-that choices are tick boxes, so the form can be filled in with a PDF viewer on a phone or computer and saved.
Can people fill it in on a phone?
Yes, in most phone PDF viewers and browsers. Or open this page, tap “Fill it in here”, and save the answer card.
Can I edit the questions?
Yes. Edit any wording, untick questions, move them up or down, and add your own as one line, two or three lines, or a tick-box choice.
Can I print it instead?
Yes. Print the PDF at 100% (Actual size). Turn on the ink saver for black line art on white, or larger text for more writing room.
Is there a version for kids?
Yes. The kids’ set asks about colours, animals, games and treats, with an allergy question for a grown-up to fill in. Larger text gives more room for young writers.
How do I collect the answers?
People send the filled PDF or the answer card back to the organiser, who passes each one to the right Secret Santa. This page doesn’t collect anything itself.
Does it work with A4?
Yes. Choose US Letter or A4, and the form is laid out for that paper.
Are my answers uploaded?
No. The questions and answers stay on your device, and nothing is saved when you close the page.
Can I use letters from other alphabets?
Yes, in the questions and on the answer card. In the PDF fields, how other alphabets show depends on the PDF viewer.
